Introduction
This process is designed for state approved trainers whose designation expired under particular circumstances and cannot reapply due to the following reasons: Trainer designation was I, II, or III prior to expiring, or GaPDS system is impeding applicant from applying for previous trainer designation due to career level requirements.
If the previous reasons apply to your case, please complete the application entirely and provide proper documentation in applicable sections. Georgia Training Approval will review the application to determine trainer designation reactivation. An e-mail notification informing the status of the review will be provided within thirty days from the date submitted.
Application Guidelines
The Expired Trainer Designation Appeal must be submitted to DECAL's SurveyMonkey Apply website. Once on the link, click Apply to begin the application. If you do not have an account already, you will be prompted to register for one. In the application form, click on Instructions for more information about the application guidelines. Once you have completed all required parts of the application, make sure to submit your application. You should receive an automated email confirmation of your submission.

In order to complete the form, you may need access to your GaPDS profile. By visiting https://gapds.decal.ga.gov/ and logging in, you can access your GaPDS profile. Your GaPDS number, the code that identifies you as an approved trainer, and training sessions you've conducted are all collected in this form.
There are two sections in this form that request for you to upload files. The accepted formats are .pdf and .doc. When you click on the Upload a File button, you should be able to submit multiple files at once. You will need to provide evidence of training sessions you've conducted within the past five years.
