This application form is for companies interested in being considered for placement on the DECAL Nutrition Services Vendor/Food Service Management Company (FSMC) List. This list serves Georgia DECAL's Child Nutrition Programs: The Child and Adult Care Food Program (CACFP) and Happy Helpings (Georgia's Summer Food Service Program (SFSP)). Organizations participating in CACFP and Happy Helpings utilize the list as a tool to identify food industry companies that provide quality meals to Georgia's children and adults in accordance with federal regulations, 2 CFR §200.318(a) and (k), 7 CFR §225.17 and 7 CFR §226.22.
The following documents are required for consideration for placement on the DECAL Nutrition Services Vendor/FSMC List:
- Two Letters of Support (notarized)
- Completed Suspension and Debarment Certification Form, which can be accessed here
- Food Service Permit and Current Health Score
- Business License
Upon review and satisfaction of a complete registration process, companies must proceed with completion of the required Nutrition Services Vendor/FSMC Information Session.
Company selection for the Vendor/FSMC List will be based upon a scoring rubric, review of required documents, good standing with Department of Early Care and Learning and any other Child Nutrition Programs and training completion.
